In October 2018, NC received federal approval from the Center for Medicare & Medicaid Services (CMS) to transition to Medicaid managed care and make other changes related to Medicaid Transformation.
This conference will explore the recent changes related to Medicaid managed care in NC and discuss impacts of social admissions on healthcare. It will also provide information related to services offered through DSS and other community resources to help facilitate effective transitions of care. Participants will be able to exchange ideas and recommendations through group discussion use of case scenarios.

Charlotte AHEC Nursing Education is an approved provider of continuing nursing education by the North Carolina Nurses Association, an accredited approver by the American Nurses Credentialing Center's Commission on Accreditation. 
This program has been pre-approved by the Commission for Case Manager Certification to provide continuing education credit to Certified Case Managers. The course is approved for 6.25 CE contact hours.
