Course Description:
The Advanced Steps Advance Care Planning (ACP) Facilitator Certification course is designed to assist participants learn the skills needed to facilitate foundational ACP conversations with individuals who are in their last few years of life. This course is intended for healthcare professionals (nurses, social workers, chaplains, physicians, advanced practice providers) and other care providers. This course is appropriate for all levels of knowledge.
Course Requirements/Pre-Requisites
(Instructions for completing the following pre-course requirements will be emailed to you upon registration for this course):
• Complete Respecting Choice ACP Core Components Course
• Complete Respecting Choices Online Modules 1-5
• Read through the following:
1. Advanced Steps ACP Conversation Guide: For individuals with decisional capacity (RC 3115)
2. CPR Decision Aid (RC 0301)
3. POLST form (appropriate for your organization/community or distributed by ACP Instructor)
