The Ambulatory Care Pharmacy Forum of North Carolina is a network for, and led by ambulatory clinical pharmacists with an ongoing goal of providing networking, collaboration, and educational opportunities for likeminded pharmacists in North Carolina with an emphasis on practical ambulatory care topics and unmet public health needs.

In addition to providing networking opportunities for ambulatory care pharmacist, this knowledge-based CPE activity will provide the pharmacist with updates on continuous blood glucose monitoring practices, an overview of cardiovascular trials with glycemic control pharmacotherapies, updates and best practices of anticoagulant and antiplatelet therapies, and strategies for incorporating the pharmacist into value-based care models.

The University Of North Carolina Eshelman School Of Pharmacy is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. The program ACPE # 0046-9999-18-019-L01-P (Pharmacist) provides 4.0 contact hours of continuing pharmacy education credit. To receive CE credit, you must complete the CE attendance form and the evaluation of the program. Statement of credit can be viewed and printed in CPE Monitor. Statements of CE Credit will be processed in approximately 2 to 3 weeks.

  • List pertinent clinical trails evaluating cardiovascular outcomes with the agents used to manage glycemic control.
