Description

This program is a hybrid with both in person and webinar attendance options as follows:

Option 1: Attend IN PERSON | Morning and Afternoon Sessions (6 hours of credit total)
Option 2: Attend VIRTUALLY or IN PERSON | Morning Session only (3 hours of credit)
**NO option to attend VIRTUAL or IN PERSON for only the Afternoon Session.

Because of the interactive nature of the afternoon session, there will not be a virtual option for the afternoon session. Participants need to attend IN PERSON.

The Ambulatory Care Pharmacy Forum of North Carolina is a network for and led by ambulatory care pharmacists. The program is intended for clinical pharmacists with an ongoing goal of providing networking, collaboration, and educational opportunities for likeminded pharmacists in North Carolina with an emphasis on practical ambulatory care topics and unmet public health needs. In addition to providing networking opportunities for ambulatory care pharmacist, this application-based CPE activity will provide the pharmacist with clinical practice updates on a variety of topics.

Objectives

  • Explain the latest guidelines for the evaluation and management of chronic kidney disease, including key updates from KDIGO 2024.
  • Identify medications used in the management of CKD, focusing on their mechanisms, benefits, and potential side effects.
  • Identify common medications that are nephrotoxic or require renal dose adjustment in patients with CKD.
  • Discuss patient-centered, evidence-based approaches to CKD management, including medication management, lifestyle modifications, patient education, and social determinants of health to enhance patient outcomes.
  • Understand the latest guidelines for the evaluation and management of osteoporosis, including key updates from ACP 2023.
  • Identify and describe the medications used in the management of osteoporosis, focusing on their mechanisms, benefits, and potential side effects.
  • Discuss patient-centered approaches to osteoporosis management, including lifestyle modifications, patient education, access to medication, and the use of medication therapies to optimize patient outcomes.
  • Explain the pros and cons of enrolling in Original Medicare vs. a Medicare Advantage Plan.
  • Summarize the Standard Part D benefit for 2025.
  • Discuss how the Medicare Part D Redesign impacts stakeholders.
  • Understand the latest guidelines and literature for the evaluation and management of obesity, including key updates from ACC/AHA/TOS and AACE/ACE.
  • Identify and describe the medications used in the management of obesity, focusing on their mechanisms, benefits, potential side effects, and medication access.
  • Evaluate the long-term use of GLP-1 receptor agonists in obesity management, including their impact on weight, comorbidities, and adverse effects.
  • Review the management of anticoagulation, focusing on medication mechanisms, benefits, side effects and monitoring.
  • Explore the latest guidelines and literature for use of DOACs in patients with obesity, cirrhosis and cancer.
  • Apply best practices for periprocedural management of DOACs.
  • Recognize resources for medication access for DOACs.
  • Understand the role of pharmacy learners in improving population health outcomes through patient outreach and identifying healthcare gaps for closure.
  • Identify successful models and strategies for integrating learners into population health pharmacy work and practice.
