Description

This program is a hybrid with both in person and webinar attendance options as follows:

Option 1: Attend IN PERSON | Morning and Afternoon Sessions (6 hours of credit total)
Option 2: Attend VIRTUALLY or IN PERSON | Morning Session only (3 hours of credit)
**NO option to attend VIRTUAL or IN PERSON for only the Afternoon Session.

Because of the interactive nature of the afternoon session, there will not be a virtual option for the afternoon session. Participants need to attend IN PERSON.

The Ambulatory Care Pharmacy Forum of North Carolina is a network for and led by ambulatory care pharmacists. The program is intended for clinical pharmacists with an ongoing goal of providing networking, collaboration, and educational opportunities for likeminded pharmacists in North Carolina with an emphasis on practical ambulatory care topics and unmet public health needs. In addition to providing networking opportunities for ambulatory care pharmacist, this application-based CPE activity will provide the pharmacist with clinical practice updates on a variety of topics.

The program ACPE#0046-9999-26-005-L99-P(Pharmacist) provides 3.0 contact hours of containing pharmacy education credit from 8:30AM to 12Noon. Objectives

  • YANG: Understand recent developments in terms of anti-amyloid therapy for Alzheimer's Disease.
  • YANG: Describe GUIDE program from Medicare Innovations Center for dementia management.
  • YANG: Learn the contraindications to anti-amyloid therapy in terms of both medications and other medical conditions.
  • WRIGHT: Identify individuals who should be screened for MASLD and MASH in the primary care setting.
  • WRIGHT: Apply NITs and biomarkers for optimal assessment, risk stratification, and monitoring of MASLD and MASH.
  • WRIGHT: Implement guideline-recommended strategies for managing MASLD and MASH, including treatment of comorbid metabolic conditions and collaboration with specialists when needed.
  • WRIGHT: Discuss evidence supporting the use of recommended pharmacotherapies with proven efficacy in MASH.
  • CAMPBELL: Describe the key provisions of CPP Law Changes and their impact on Clinical Pharmacist Practitioner (CPP) scope of practice, including expanded authorities for drug therapy management in population health, test ordering, and collaborative practice agreements, and identify essential compliance steps for maintaining CPP licensure and patient safety.
  • CAMPBELL: Analyze implementation strategies employed by large North Carolina healthcare institutions in response to the updated CPP Act, focusing on integration of expanded services into team-based care models.
  • CAMPBELL: Discuss potential advances in pharmacist-provided clinical services and explore practical applications of CPP roles in chain and rural pharmacies to address population health management and access gaps in underserved communities.
  • PANEL: Explain the basic components and functions of insulin pumps to enhance pharmacists’ understanding of the technology.
  • PANEL: Identify the clinical benefits and potential challenges of insulin pump therapy in diabetes management within primary care.
  • PANEL: Apply practical strategies for pharmacists to support and optimize insulin pump use in patients’ diabetes care plans.
  • KAAN: Describe the pathophysiology and diagnostic criteria for LADA.
  • KAAN: Discuss management strategies for LADA.
  • KAAN: Review the role of adjunctive antihyperglycemics in LADA treatment.
  • HERRING: Briefly review the 2025 AHA HTN guidelines.
  • HERRING: Briefly review resistant HTN.
  • HERRING: Briefly review CLICK trial data (chlorthalidone in pts with stage 4 CKD) vs 2025 AHA HTN guideline recommendation.
