The Design Day is an interactive, hands-on experience where community partners come together to bring the 2025 Community Health Assessment (CHA) findings to life. Through shared learning, reflection, and collaboration, participants will blend data with lived experience and on-the-ground knowledge to identify cross-cutting themes, strengths, and barriers shaping health outcomes. Together, we’ll begin co-creating early ideas and recommendations to help guide the priority goals and action areas of the 2026 Community Health Improvement Plan (CHIP).

Why Attend Design Day?
Design Day is more than a meeting—it’s a chance to help shape the future of community health in Mecklenburg County. This interactive, hands-on experience brings community partners together to turn 2025 Community Health Assessment (CHA) insights into meaningful action. Participants won’t just review data—they’ll work alongside peers across sectors, blending data with lived experience and on-the-ground knowledge to explore shared challenges, uncover strengths, and identify opportunities for impact. Through guided design activities, reflection, and collaboration, attendees will help surface cross-cutting themes and begin co-creating early ideas and recommendations that will inform the 2026 Community Health Improvement Plan (CHIP).

Design Day is ideal for those who want to:
• Move beyond discussion and actively contribute to solutions
• See how data, community voice, and lived experience connect
• Build stronger relationships with partners working toward shared goals
• Help shape priorities that will guide community health efforts moving forward

By the end of the day, participants will leave with a clearer understanding of community health needs, stronger cross-sector connections, and a shared commitment to next steps in the CHIP co-design and implementation process.

Design Day Objectives:
• Engage in collaborative, hands on activities designed to deepen understanding of key findings from the 2025 Community Health Assessment (CHA) and apply them to real world community health challenges.
• Contribute lived experiences, professional expertise, and organizational perspectives to identify shared priorities across all sectors represented
• Analyze community health needs by integrating data insights, community voice, and on the ground knowledge to build a collective picture of current opportunities and gaps.
• Identify cross cutting themes, strengths, and barriers that influence health outcomes across diverse communities and populations.
• Co develop early stage ideas, recommendations, or prototype strategies that will inform the priority goals and action areas of the 2026 Community Health Improvement Plan (CHIP).
• Strengthen collaborative relationships among community partners by working together in group activities, facilitated design sessions, and collective reflection
• Commit to next steps in the CHIP co design and implementation process by understanding how their ongoing participation will support community health improvement moving forward.
