Target Audience

Target Audience: Nurses, care managers, case managers, social workers, clergy, advanced practice providers, physicians and all health care professionals who assist with advance care planning.

This workshop is jointly provided by South Piedmont AHEC Nursing Education and Your Care-Your Choice, a department of Atrium Health.

Course Description:

This evidenced-based course is designed to develop skills to initiate advance care planning and goals of care conversations with anyone facing serious illness and frail elders to make timely, proactive and specific end-of-life decisions. This course builds on the communication and interview skills learned in the Core Component used for person-centered advance care planning (ACP) facilitation. Participants will learn how to convert these conversations into medical orders using the MOST (Medical Orders for Scope of Treatment) form that can be followed throughout the continuum of care.

Course Requirements/Pre-Requisites

(Instructions for completing the following pre-course requirements will be emailed to you upon registration for this course):

• Complete the Respecting Choice ACP Core Component Course, including Core Curriculum Modules 1-4
• Complete Respecting Choices Online ACP Facilitator Module 5
• Read through the following:

1. Advanced Steps ACP Conversation Guide: For individuals with decisional capacity (RC 3115)
2. CPR Decision Aid (RC 0301)
3. POLST form (appropriate for your organization/community or distributed by ACP Instructor)

Learning Outcome:

Participants will be able to demonstrate effective communication skills and techniques for facilitating person-centered conversations for those living with advanced medical conditions and frailty for both in-person visits and telehealth.

Objectives

  • Define benefits of First Steps conversation and identify appropriate target population.
  • Demonstrate the application of the person-centered decision-making framework to elicit preferences and decisions.
  • Create strategies to transfer individual goals, values, and preferences to a written plan.
  • Demonstrate beginning competency in facilitating First Steps ACP conversations through role-play exercises.
